Tasks/Assignments for intern: Job Description: Under supervision of the Guardianship Supervisor, the guardian case manager will oversee and coordinate legal, medical, financial, social, and psychiatric case management services fulfilling the role and duties of Guardian for diverse populations of individuals with varying degrees of disabilities and challenging life circumstances who have been declared wards of the State or County Specified. Interns will work closely with Guardianship case manager to assist with the responsibilities of maintaining wards care and case. Cities, Counties, Regions and territories: • GFS will work with interns and ensure that they are only assigned cases within 50 miles radius of their resident/school • Provisions will be made to accommodate interns unable to meet at GFS office, with alternative sites to be identified for meeting purposes Major Responsibilities: GFS guardian representative will maintain the lead on every case with assistance from interns • Assist with the completion of ward guardianship intake packet once referral is received • Assist with processing and completing court documents required for wards incompetent hearing, make sure documents are provided to staff scheduled for hearing • Intern is welcomed to attend ward court hearing if desired • Intern assist with making contact the ward upon 72 hours of guardianship assignment. • Intern assist with conducting visitation with ward with five (5) business day upon ward assignment • Intern assist with ensuring Guardianship appointment letter with SEAL is scan into wards electronic file • Intern assist with forwarding ward Guardianship appointment letters to appropriate supports (group home, SSA, Medicaid, MCO and etc.) • Intern assist with development of ward’s care plan • Intern assist with ensuring phone contact is made with ward directly and or care provider and support teams on a monthly basis, and document contact in SEMS • Intern assist with conducting home visitation with ward in accordance with GFS rule every 60 days, and document contact in SEMS • Intern assist with monitoring the living condition of assigned wards to ensure that he/she is receiving care, comfort, and supervision, and that their basic needs of food, shelter and medication (if needed) are met, whether the ward resides alone, in a group home, a nursing home or in a more restrictive facility. • Other duties as assigned REQUIRED DOCUMENTATION: Interns may assist with completing the required documentation for wards • NOTES • PCP • Quarterly Assessments • STATUS REPORT • Critical Due dates Minimum Education/ Experience: • Four (4) year college degree in human services or a related field and at least three (3) years of post-graduate case management experience in social services setting. Must meet the standards of a Qualified Professional. • Knowledge of principles, practices, and techniques of case management and administration, case management documentation and records management. • Ability to maintain the highest standards of ethical behavior, exercising honesty and integrity, respect, confidentiality, and fairness in the execution of their official responsibilities. • Ability to establish and maintain effective relationships with residential facilities, care providers, family members, benefit carriers, government officials and departments, community partners and resources, and the general public. • Ability to communicate detailed and often sensitive information effectively, both orally and in writing. • Ability to maintain confidential information. • Skill in emphasizing, valuing, and dealing successfully with the special capabilities, distinctive cultural histories, and unique needs of people of various socio-economic, ethnic, and cultural backgrounds. • Skill in self-discipline, dependable, and ability to work independently, manage multiple projects, and prioritizing multiple tasks and demands. • Skill in operating a personal computer, utilizing a variety of standard and specialized software including electronic case management and client financial systems.
