The criminal background check must include any felonies and misdemeanors from the time the you were 18 years of age to the present or for the past 7 years, whichever period is shorter. The social work liaison at your institution may be able to assist you with completing this task.
To avoid delays to your computer access, complete and return the Intern information form at least five business days before your placement starts. The Confidentiality Agreement, immunization guidelines, Intern Information Form, and directions to Duke are attached. Online training sessions for Compliance, Fire/Safety, HEICS, End-of-Life-Care, HIPAA and Service Recovery will be arranged after your ID and PC access are established. The TB test should be dated one month or less before the start of your internship.

Tasks/Assignments for intern: • Shadow clinical social workers from the Medically Ill Child & Adolescent Psychiatry Clinic, and assist with case management
• Co-facilitate weekly therapy group
• Attend monthly Transition Taskforce meetings
• Attend ATLAS mentor meetings and assist in planning the monthly meetings
• Facilitate ATLAS monthly meetings
• Follow up with ATLAS families via phone or in person regarding their transition experience
• Attend ATLASfit Meetings, as well as program planning meetings
• Assist with program planning for programs such as Campference and Duke Children's Prom
Ideal student description: The ideal student for a placement with our agency would be someone who is flexible, mature, and is excited about working with adolescents and young adults who live with various chronic illnesses.
