Other Fields of Practice: As an MCO/LME we also are involved with other teams to assure the quality of provider services in Wake County. Plus we are part of the Transition to Community Living initiative as part of the recent DOJ settlement. And we are creating an Integrated Healthcare Team with Community Care of North Carolina. There is a lot going on
Major Functions: Administration, After Care/Follow Up, Assessment, Case Management, Client Empowerment, Crisis Intervention, Data Collection, Discharge Planning, Documentation, Evaluation/Research, Information/referral, Multidisciplinary work, Team Meetings, Treatment planning
Other Functions:
Tasks/Assignments for intern: BSW interns will be serving primarily as Case Managers, linking and monitoring high risk/high cost consumers who have multiple crisis/inpatient/jail stays related to their MH/SA Diagnosis. Assisting consumers directly to assess/reduce barriers to services. Work with providers to assure engagement. MSW students will have opportunities to work with Alliance Liaisons in ED's/Crisis/Inpatient to assist in assessing service needs and to work with complex cases where services gaps exist and multiple needs impede engagement in services.
Ideal student description: Energetic, macro and micro focused, knowledge of MH/SA issues, creative, willing to ask questions but willing to put forth his/her own ideas. Flexible as tasks can change based on crisis/emergency needs.
