Work Plan/Evaluation for MSW Advanced Practicum Internship SW 654
Directions for completing Work Plan:
This evaluation is to be completed at the end of the spring semester. Practicum hours should be completed each semester before the last day of final exam period but no sooner than the last week of classes. Students and practicum supervisors should complete the evaluation separately using their work plans to guide their respective ratings on each of the behaviors. Subsequently students and practicum supervisors should meet to discuss the evaluation prior to submission. The practicum supervisor will, using the form below, rate students on each of the behaviors. The supervisor will then make recommendation to the faculty practicum liaison for final S/U grade. Based on the feedback from the student, practicum supervisor, the faculty practicum liaison will assign a grade for the student.
How to rate the behaviors: [(3-5) is satisfactory (S) and (1-2) is unsatisfactory (U)]
This Likert scale is to measure your assessment of this student in light of what one could expect of a well-trained and high-performing MSW student. The scale is not to assess the intern in light of expectations for a seasoned professional social worker.

4 | Above Average The intern is functioning above expectations in this area, as demonstrated by behaviors. |
3 | Competent The intern has met the expectations in this area, as demonstrated by behaviors. |
2 | Needs Improvement The intern has not yet met the expectations in this area through demonstration of behaviors, but gives indication they can do so in the near future. |
1 | Unsatisfactory The intern has not met the expectations in this area through demonstration of behaviors, and does not give indications they can do so in the near future. |
