Work Plan/Evaluation for MSW Advanced Field Internship SW 654

Directions for completing Work Plan:

This evaluation is to be completed at the end of the spring semester. Field hours should be completed each semester before the last day of final exam period but no sooner than the last week of classes. Students and field supervisors should complete the evaluation separately using their work plans to guide their respective ratings on each of the behaviors. Subsequently students and field supervisors should meet to discuss the evaluation prior to submission. The field supervisor will, using the form below, rate students on each of the behaviors. The supervisor will then make recommendation to the faculty field liaison for final S/U grade. Based on the feedback from the student, field supervisor, the faculty field liaison will assign a grade for the student.

This Likert scale is to measure your assessment of this student in light of what one could expect of a well-trained and high-performing MSW student. The scale is not to assess the intern in light of expectations for a seasoned professional social worker.
