Work Plan/Evaluation for MSW First-year Field Internship SW 652
Directions for completing the end of semester evaluation:
This evaluation is to be completed at the end of the spring semester. Field hours should be completed each semester before the last day of final exam period but no sooner than the last week of classes. Students and field supervisors should complete the evaluation separately using their work plans to guide their respective ratings on each of the behaviors. Subsequently students and field supervisors should meet to discuss the evaluation prior to submission. The field supervisor will, using the form below, rate students on each of the behaviors. Comment boxes are provided for each competency. However, only ratings of 1 (unsatisfactory) or 2 (needs improvement) require comments. The supervisor will then make recommendation to the faculty field liaison for final S/U grade. Based on the result of feedback from the student, field supervisor, and the completed evaluation form, the faculty field liaison will assign a grade for the student.
How to rate the behaviors: [(3-5) is satisfactory (S) and (1-2) is unsatisfactory (U)]
This Likert scale is to measure your assessment of this student in light of what one could expect of a well-trained and high-performing MSW student. The scale is not to assess the intern in light of expectations for a seasoned professional social worker.
